Information for Patients and Families
Cancer is presently designated as a reportable disease in the State of Utah (Cancer Reporting Rule, R384-100). As such, all cases of cancer diagnosed or treated in the state must be reported to the Utah Department of Health. Through a letter of agreement between the Utah Department of Health and the University of Utah, the Utah Cancer Registry is designated as the official repository for such data. If you are a cancer patient interested in obtaining your information from the UCR, or are a next of kin, legal guardian, or have power of attorney for an individual who had or has cancer, you can do so by sending a notarized Request for Confidential Cancer Record Form.
